As our Well Preserved group is rather large, many of your neighbors will be wearing their badges to our events.  If you are interested in obtaining a Well Preserved name tag, please complete the form below for each name tag. ​These are available only to those who are confirmed neighbors in The Preserve and who have been set up on our web site.

Note: Since the name tags are magnetic, those with pacemakers should wear it on the opposite side from the pacemaker.

The cost of each name tag is $8 and only those prepaid will be ordered.  Payment can be made by either cash or a check made out to Beth Cullum may be left in the box by her front door or a check may be mailed to her at:
